It looks like someone stepped on shifter boot and might have ripped the boot out of the screws. Can you reinstall them. (might have to drill a few new holes to secure the ripped area.)

I'll pull it out of one gear, then there is a slight "nod to the gods" (maybe a 1/2 second pause) in neutral, then pull it into the next gear. I also find that shifting gears at RPMs between 2,500-4,000 makes the smoothest shifts for me.
